WELLNESS AND WRAPAROUND SUPPORT

We support the whole person through a comprehensive wellness and wraparound model grounded in the Eight Dimensions of Wellness. This approach addresses emotional, physical, spiritual, social, financial, occupational, intellectual, and environmental well-being while providing personalized wellness planning tailored to each individual’s needs.

Our model ensures every person is surrounded by a coordinated network of care. We connect individuals to the emotional, practical, and community resources essential for stability and long-term well-being. We advocate on their behalf, collaborate directly with service partners, and provide consistent follow-through so no one has to navigate challenges alone.

Our wraparound support includes:

• Food and hygiene assistance
• Clothing support
• Transportation support
• Housing referrals
• Financial guidance
• Mentoring
• Recreational space
• Coordinated referrals

Through this approach, we help individuals access the services and support that strengthen stability, healing, and long-term success.
